BILLING / INSURANCE / SLIDING SCALE 

Self Pay: 

  • General Appointments & Physical Exams - Patients without insurance that pay in full on the day of service will receive a 35% discount for the office visit expense. (Note - we are unable to discount lab fees or procedure fees.
  • Sports Physicals - Self pay cost is $58 and is required to be paid in full on the day of service. If you have insurance coverage for your child, call your provider prior to the appointment and check for "Well Child Check" eligibility, as we can do a full Well Child Check and Sports Physical together and bill insurance if s/he is eligible.

Insurance Billing: 

  • MMC will bill your insurance if we are contracted as a network provider. 
    (Please contact your insurance company to receive a list of your network providers.) 
    • If we are not listed as a network provider, we ask that you self pay for your office visit, and we will provide you will all the necessary paperwork for you to send to your insurance company for re-imbursement.  
  • We are unable to accept out of area insurance if you are traveling through this area and do not maintain an address in this area. 
    • We require that your office visit be paid in full on the day of service, and we will provide you with the appropriate documents to submit to your insurance company for reimbursement.  

Medicare  /  Medicaid: 

  • Please note: At this time, Mountain Medical Center is accepting new Medicaid patients who live in Ouray County ONLY. We apologize for the inconvenience. We are also not accepting anyone with Medicaid Prime.
  • If you live in Ouray County or are already established with us from out of county, please contact Rocky Mountain Health at 970.244.7912 to switch to Rocky Mountain Medicare and 888.367.6557 to switch to Rocky Mountain Medicaid. Many specialist providers will only accept referrals if you have switched to Rocky Mountain Medicare or Rocky Mountain Medicaid. A simple phone call can get your coverage converted and make it easier for you to be referred to specialists. Sliding Scale: 

  • Your health care maintenance is of utmost importance to us.  We also realize that financial circimstances can sometimes prevent people from seeking medical care.  If you are experiencing a difficult financial situation, you may be eligible to qualify for "Sliding Scale" or discounted fees for medical care. Sliding scale determination is based on the NHSC guidelines and requires income verification.   Verification will typically include last year's tax return and the previous month's pay stubs for all wage earners in the household. 
  • Eligibility may also be based on current participation in certain federal/state public assistance programs which may include Social Security Disability income, Temporary Assistance for Needy Families, Free or Reduced School Lunch Program, or other public assistance programs.  
  • Eligibility will be re-evaluated on a semi-annual basis. All paperwork must be completed and returned to the office at least 48 business hours PRIOR to the date of your appointment for verification of eligibility.
