Many of you may be unaware that we have a fantastic local resource for medical questions and care called the Tri-County Health Network. Per their website, the TCHNetwork is a vertical network of providers committed to improving the quality and coordination of health in our tri-county area by increasing access to healthcare and health services at lower costs.

Community Health Workers
Community Health Workers provide biometric testing to screen underserved people for risk of diabetes and heart disease and offer 1-to-1 peer support to engage people in managing lifestyle changes to improve overall health.

Chronic Disease Outreach & Prevention
Patient Health Navigators, aligned with our partner clinics, share the goal of improving the overall health of patients with diabetes and heart disease by promoting healthy behaviors and encouraging self-management of chronic disease to improve the patient’s overall quality of life.

Entitlement Assistance & Insurance Assistance Programs
Assistance in enrolling eligible children and their families in entitlement programs, such as Medicaid, Colorado Health Plan Plus (CHP+) and other similar programs. Navigators are Certified Enrollment Assistants. Our Health Coverage Guides can offer individual navigation of the Colorado Health Insurance Benefits Exchange or “Marketplace” as well as education on the Affordable Care Act & how it effects individuals as well as small businesses.

Regional Medical Shuttle
A non-emergency shuttle service providing area residents with transportation to medical and dental appointments in Montrose and Grand Junction.

San Juan Kids Cavity Prevention Program (Skippy)
A mobile school-based cavity prevention program serving children ages 1 – 12. Administered twice a year by dental hygienists and Navigators, services include oral screening, cleaning, fluoride treatment, and sealants. Services are offered at no-cost to children.
